Meeting notes to action items

Here are my raw meeting notes:

Produce: 1) One-paragraph summary of what was discussed, 2) Decisions made (bulleted, decisive language), 3) Action items as a table with columns: Action / Owner / Due date. Infer owners and due dates where possible — if you have to guess, mark with (TBD). 4) Open questions that need follow-up. 5) Anything in the notes that seemed important but wasn't resolved.
